Lead Form QR — capture name, email, phone in one scan
Build a custom intake form, share via QR. Webhook + email + CSV export.
Lead Form QR
A Lead Form QR opens a customisable form on the scanner's phone. They fill it out; you get an email + CSV row + optional webhook.
What you can capture
- Name (single field or split first/last)
- Email — validated
- Phone — country-code aware
- Company + role
- Custom dropdowns (e.g. "Which product?")
- Multi-select tags
- Free-text message
- Hidden fields (UTM, source QR slug)
Where leads go
- Email — formatted notification to your nominated address
- CSV — rows accumulate; export from dashboard
- Webhook (Pro+) — POST to your CRM, Zapier, n8n
- Native CRM (Business+) — direct push to HubSpot, Salesforce, Pipedrive
Anti-spam
- reCAPTCHA v3 invisible — the scanner doesn't see it; bots get rejected
- IP rate limiting — 5 submissions per IP per hour
- Honeypot field
Use cases
- Sales-team business card with "Get a quote" form below the vCard
- Event booth QR — "Get the deck"
- Real-estate sign QR — "Schedule a viewing"
- B2B landing page QR — "Talk to sales"
GDPR-aware
The form template includes the standard "QR NFC Tap processes this on behalf of [you]" footer + link to your privacy policy. You're the data controller; we're the processor. See our DPA for the contract.
